Jay returns to the abandoned hospital alone. While exploring the maintenance tunnel the Hooded Man led him to in Entry 58, Jay finds a folder containing some of Tim's old medical records near a "slendy doll". Jay then encounters the Operator in the tunnel and quickly leaves, taking the doll and the records with him.
[00:10] Walking on a wide path surrounded by tall grass. Birds can be heard in the background.

I decided to return to the abandoned
hospital to look around some more.
[00:20] The camera pans over the grass to the left.

I would have preferred not to go alone but
after what happened with Tim, I didn’t
have much of a choice.
[00:24] Camera resumes looking forward.
[00:30] Cuts to footage of a building surrounded by trees. Jay walks in and turns left into a long hallway partially outside.
[00:48] Cuts to an interior hallway, badly damaged by fire. Jay pans up and to the left, examining the burt ceiling.
[00:54] White text appears over footage.
While looking, I think I found the room where
the fire Tim mentioned started.
[00:55] Jay continues walking and enters a room to the left. There is a window and a hole in the ceiling, and the floor is black from fire.
[01:05] Jay touches and pulls off a piece of the burnt wall and rubs his fingers together.
[01:10] He begins to walk slowly into the room and stops.
[01:16] Cuts to footage of a badly charred wall. Jay walks to it and reaches out a hand to touch it.
[01:22] He turns to the left and then to the right.
[01:24] Jay walks over an ashy floor and looks at the peeling yellow paint on the walls.
[01:36] Cuts to a wall with a hallway. A graffiti smiley face is on the left and a light switch on the right. Jay turns left and looks down a long hallway with alternating brightness and darkness.
[01:42] He zooms in and turns left to the doorway outside. He zooms back out. Outside, the brick wall of the building is visible, as are metal railings and trees.
[01:45] Jay looks down, then turns back to the first hall.
[01:51] Jay is walking down a more lighted hallway.
[01:53] White text appears over footage.
Before going to the tunnel, I went to
check the room I saw the hooded
person come out of.
[01:55] Jay walks past various doors, most marked with Xs and graffiti.
[02:09] Jay turns left into a room. There is a tiled wall marked with a spray painted dollar sign to the left. The floor is covered in various wood debris. Jay turns away and continues to walk.
[02:11] He passes more doors and slows down when an exit comes into view.
[02:36] Jay turns left into an open rooms with windows.
[02:40] He looks around then walks across the hall to a room with peeling paint and debris on the floor.
[02:45] Jay turns to the left wall, where the words “HE IS A _Liar_” are written on the wall. Jay walks toward the wall.
[02:51] He turns around, and sees the words “Follow ME” written on the opposite wall.
[02:56] Jay walks toward this wall then exits the room.
[03:07] Jay exits the building and walks in the grass.
[03:18] Cuts to the outside of a brick building with plant life growing on and around it. He walks into a room with pipes.
[03:27] Jay turns to a hole in the ground with various pipes sticking out of it.
[03:32] Jay turns on his flashlight and shines it down the hole. It does not appear to be very deep.
[03:38] The camera points up and Jay sets it on the floor.
[03:46] Jay slides into the hole from the right. He is wearing his usual green jacket and brown hat.
[03:51] Jay looks around then picks the camera back up. The ground is chest level from where he is standing.
[03:55] He crouches down, and the camera points down the tunnel.
[04:03] Cuts to Jay’s face looking down the tunnel. He is looking around. Something is regularly hitting the side of the tunnel as he crawls.
[04:13] Cuts to the camera facing the tunnel. Jay is moving more slowly.
[04:26] Cuts to another part of the tunnel, at what seems to be the end.
[04:40] Jay stops and then continues.
[04:49] Under a series of black pipes to the left, the tunnel continues. He shines his light down it.
[04:56] Jay begins to attempt getting over the pipes.
[05:03] He makes it over the pipes.
[05:05] Cuts to further down the tunnel.
[05:16] Jay stops and a thumping sound can be heard. He tracks the sound overhead, first in front of him, the behind him.
[05:27] The sound grows quieter and Jay turns back to his original direction.
[05:42] Jay begins to walk further.
[05:46] Cuts to a part of the tunnel with many pipes blocking the path. Jay sighs.
[05:55] Jay looks past the pipes and sees that the tunnel continues. He turns around, then back.
[06:03] He turns left and walks. He occasionally breathes as if he is in pain.
[06:13] Jay points the camera forward to the end of the tunnel and stops. Many pipes intersect at it. A named muslin doll rests on one pipe on the right.
[06:16] He zooms in on the doll. It has two small eyes but is otherwise plain.
[06:22] Jay zooms back out and walks toward it, looking down and sounding like he is out of breath.
[06:32] He stops and looks at the manila folder resting on pipes beneath the doll. The Operator symbol is drawn and the word “LiAr” is written in thick marker.
[06:36] Jay sets down the flashlight and opens the folder to the Pediatric Admission Profile
[06:49] He zooms in and out. It is dated 12/20/95. The name on the upper right corner is Timothy W. The rest of the last name is hidden under black marker. Jay briefly looks over the rest of the page. He flips through the other pages.
[07:05] Jay begins to put away the folder and the camera points into darkness. The thumping sound can be heard again.
[07:12] The camera looks back toward the doll, out of focus. It returns to focus and Jay turns around, trying to track the sound.
[07:22] The sound stops and Jay adds the doll to his bag.
[07:36] Cuts to the barrier of pipes.
[07:43] Jay struggles under them and stops.
[07:47] He realizes it’s the wrong path, and goes back under.
[08:00] Jay continues through the tunnel, breathing deeply.
[08:06] He stops and turns.
[08:10] High audio distortion begins as he turns, as does visual distortion.
[08:16] Jay turns back and tries to leave through the tunnel he’s in.
[08:20] He turns, and sees The Operator’s face. His face reappears through distortion as Jay tries to get away. Jay struggles to get through the tunnel.
[08:25] A low beep can be heard, and several seconds later.
[08:39] Jay makes it to the other pipes. He looks them up and down and turns. The audio distortion becomes louder.
[08:45] He slides the camera under the pipes and army crawls under.
[08:51] He picks the camera back up and moves on. The camera is shaking and pipes can be seen.
[08:58] Visual distortion takes up a third of the footage near the top. Then white, gray and black pixels take up part of it. The audio distortion is lower and more static during these.
[09:00] The audio distortion returns to the high screeching, and light can be seen.
[09:02] Jay sets the camera out of the tunnel facing away and all distortion resolves.
[09:05] Jay’s feet appear in front of the camera and he picks it up, running out of the building.
[09:13] The scene fades to black.
[09:18] White text on black screen.
I don’t know how I made it out of
that tunnel alive. Everything after
finding the medical documents
is a complete blur to me.

Someone else was in the building while
I was down there. I kept hearing
footsteps above me.
[09:38]
Fortunately, I managed to get out with
the medical documents and the doll
that were down there.
[09:48]
I’m going to take some time to look
through everything and try to figure
out what to do next.

I never want to go back to
that place ever again.
Main Theories and Observations
Since Tim has a long history of purported hallucinations, perhaps he could see the Operator in Entry 54 but ignored him on purpose because he had come to believe that the Operator was a hallucination.
Further Speculation
The "Slender-Doll" could be a ward against The Operator, or (considering what happened following its discovery) a way for The Operator to track people with ease. ~Dr SuperGreg
I'm guessing it's just totheark's way of telling Jay, "Hey, Operator-related stuff going on here, better pay attention." ~Filby
For anybody interested, I've written down a theory about this in the Theories page called Tim and the hospital.
Basically I'm saying that Tim was an inmate at a medical research center, and burned it down, making him go crazy. Sounds a bit weak like that, but will make more sense if you read the full theory (it's not stupidly long)
In this entry he discovered Tim's files in a burnt hospital. I think Tim's bad cough could actually just be physiological since he remembers being inside the burning building.
If so then the Operator could have been looking for him for years after the fire incident until he finally found the students filming Marble Hornets in the forest. ~Bangaafan
I'd say that if anything, this video has the best evidence that the Operator teleports, by the way he got into the tunnel. With his height, it would have been impossible, if not incredibly ridiculous, for him to have crawled. Also, given the new information that Tim is lying, the question arises: Why is he lying? The answer will probably be more apparent when we see what is on those medical documents. ~Hydrolaze
Does it seem somewhat strange to anybody else that apparently Hoody put Tim's files there, when there was already the slendy doll, or, if it wasn't there, that he would put a slendy doll right next to them if he was TTA? As SuperGreg put it, if it was a ward, then it either failed or WASN'T a ward, seeing as how the Operator still showed up. If it was some form of "tracking", then why would Hoody put the files there or the doll with the files if he's TTA? Also, there appears to be some sort of "eyes" on the slendy doll. Could this actually be a way for him to see, similar to the theorized camera eyes? My two thoughts here would be that either TTA is trying to get rid of Jay, because he has outlived his usefulness now that he has pretty much lost all ties, or that Alex may actually be impersonating Hoody, trying to draw Jay to the Operator to dispose of him. Jay himself said he doesn't know how he got out of there alive. It would be much easier to impersonate Hoody than to impersonate Tim as Masky. It just seems as if a cog is being thrown into the puzzle/theory of Hoody being a part of TTA. If he is TTA (Reference seems to, well, reference findings in Entry 60), then we have to wonder how come he put the files in a place so hard for Jay to reach and so easy for him to be trapped/hunted in… Everything seems fishy right now ~Jcal94
The doll is neither a ward nor does it attract The Operator. In the two instances of using the doll it has been an indicator. First an indicator that danger was close, as Masky was in the house with Jay and in the second instance it indicated the room that Jay would have to enter to stop the house teleporting him around. In this entry ToTheArk is using the doll for exactly the same purpose, to indicate danger, that The Operator or slender-minions are around. Unfortunately Jay hasn't realised just yet. ~Jimi06
The SIR. A few interesting things I noted while watching this video. The Operator Symbols seen in 58 have been crossed out with red. Also, the doll is all wrong. The legs are stretched, but not the arms. Perhaps thiswas TTA's way of mocking The Operator if it or Alex found the documents. I am assuming that the person walking around was Alex. As for the Operator, doesnt it seem odd that it's suddenly reapeared after all of these months? It was last seen, chronologically, in Entry 52.
One last thing. What is the significance of showing us that burnt room. I've been thinking, and I think i've figured out the connection. Operator and hospital. I think this is perhaps where the Operator, became the Operator. The fire scorched its face off and deformed its arms. I know this doesnt agree with anything we've seen before, but the name is an odd coincidence, and i can't think of another reason why that room would be important.
Hmm…maybe the Operator has some kind of connection with that hospital, like he used to work there and perform "operations". It would fit that "operator" title. Just a thought though…
I think a more important question is why Slendy didn't stupefy and mind wipe Jay in the tunnel like he has before? He was certainly close enough. Maybe the doll protects against Slendy's powers.
Wrote that transcript and I'm exhausted! But I'm curious about the footsteps. I mean, I think we can assume it's not The Operator since he would usually teleport, but maybe I'm wrong. Maybe he was exercising to keep up his figure (sarcasm…). So probably Hoody? But why would he need to be there? The folder and doll were already there, what else would he need to do? Also, maybe I'm wrong, but the way back didn't seem to take the same path as the way there. Sure, could just be because part of it was cut out, but could be the slender doll working its teleportation powers… (Tim OOG calls it the stop-motion doll) -Kaylee2014
